New Dental Insurance Plan, Vision Insurance Plan and Hearing Aid Discount Plan For GAE-R Members!
GAE-R is proud to announce the availability of new dental and vision insurance plans for GAE-R members. The plans are underwritten by leaders in their fields, with the dental plan underwritten by Security Life Insurance Company of America (or its insurance partner, Symetra Life Insurance Company in select states) and the vision plan from Vision Service Plan.
The GAE-R Dental Insurance Plan allows you complete freedom to choose any dentist you wish. There are two plan designs to choose from with an option for up to $2000 in annual benefits. Coverage available for individuals, husband and wife, parent and child(ren) or family, and you can pay the premium in a variety of methods, including automatic bank debits from you checking or savings account, by credit card, or by direct bill on a quarterly, semi-annual or annual basis. The earliest effective date for coverage under the GAE-R Dental Insurance plan is November 1, 2007.
The GAE-R Vision Insurance Plan provides eye exams, lenses (including contact lenses) and frames once every 12 months. The plan operates through participating doctors in the VSP signature network of 32,000 national access points. The plan will also pay benefits if you choose an out-of-network provider. Enrollments received after November 1st will not be effective until the next plan year, e.g., enrollments received after November 1, 2008 will not be effective until 2009.
GAE-R Members also have access to a national discount hearing aid plan from American Hearing Aid Associates. Unlike the dental and vision plans, this is not an insurance plan. It is provided at no cost to you to access reduced rates from network providers. The GAE-R hearing aid discount plan is available immediately.

A full explanation of the dental benefits, exceptions and limitations is contained in the Certificate of Insurance under Group Policy Form GH-1112-38940 issued to the Voluntary Group Trust (For Symetra, policy form LGC-8854-38990). Benefits may vary by state. The Dental Plan may not be available in all states. No agent has the authority to change any benefits, to bind coverage with Security Life Insurance Company of America, or to promise a certain effective date. The vision plan is subject to all conditions and limitations as listed in the group contract issued by Vision Service Plan to the Georgia Association of Educators.
